understanding the health care system

Why is it vital to include the local health care system when designing an EBP implementation? Do research and seek anecdotal evidence from your course mentor that you will use for your personal change project?

Evidence-based practice is crucial in the healthcare system because it is explicit, conscientious, and aids in obtaining the best evidence when making decisions about a patient's care. EBP is the incorporation of clinical expertise, patient values, and the best research evidence into a patient's decision-making process (LoBiondo & Haber, 2005). Heart failure, asthma, and diabetes are all examples of these practices. Some of the examples of EBP include critique and synthesis of evidence, discharge instructions for the patients suffering from heart failure, implementation, and evaluation of the patient care and the provider performance, with consideration of the practice to be implemented (Melnyk, & Fineout-Overholt, 2011). EBP is governed by the legal and ethical issues, the right to personal determination, confidentiality, equality and fair treatment, right to privacy and dignity as well as the protection from harm and discomfort. The important and general criteria in the healthcare system should be taken into consideration when implementing the EBP (Anderson, 2006). The healthcare system is made up of patient care, human resource management, hospital infection control, information system management and quality improvement. The key elements to implement in the EBP include questioning the practices of the nursing experts, integrating the EBP with a higher standard such as competencies for EBP (Gibbs, 2003). Moreover, the tools for enhancing EBP such as educational class time and meetings should be included, constant recognition of the EBP and higher life support and the ability of the leaders to shape the EBP model.

2. Compare and contrast two change theories, and determine which theory makes the most sense for implementing your specific EBP project. Why? Has your mentor used either theory and to what result?

Evidence-based Nursing is grounded on the integration of the best research evidence with professionalism and respecting the values of the patient in order to facilitate a clinical decision making. This is a critical part of quality improvement in the nursing practice. Nurses play a major role in facilitating and using the evidence so as to become accountable for patient care in an outcome-oriented practice (Jeffcoate, Game & Cavanagh, 2005). Nurses are trained in evidence-based practices and hence have access to these skills. Since the implementation of EBP the nursing profession are paramount, most organizations seek for nurses who can analyze the existing practices, and separate the alternatives from the differing information (Calcutt, 2002). They use EBN to improve the outcome of the patient. Neuropathic diabetes requires therapeutic regimens which are focused on the various causative elements such as neuropathy, revascularization, blood sugar control and the prevention strategies. The diagnosis of diabetic neuropathy is challenging and is confirmed by the background, clinical examination as well as the Semmes-Weinstein filament examination (Aarons, Hurlburt, & Horwitz, 2011). Medical therapy for neuropathy, improvement of the vascular flow and the surgical decompressions contribute to effective management. The Achilles tendon increases the procedures, for instance, they are shown to be improving the rate of healing the neuropathic foot plantar ulceration through the reduction of focal pressure effects and improvement of kinematics.

The annual foot examination is recommendable to all persons suffering from diabetes so as to identify the high-risk foot condition such as the peripheral vascular insufficiency, loss of protective sensation as well as the structural foot deformities (Oman, 2010). The specific interventions are best suited for such people since they help to reduce the amputation risk.
