Decreased cardiac output due to changes in conduction, rate, and rhythm as expressed by 125bpm irregular pulse, apical heart rhythm, pitting edema, and disorientation -Patient will have adequate cardiac output as evidenced by Bp, heart rate, and rhythm being within normal ranges.
Keep an eye out for an increase in heart rate, blood pressure, weariness, and pain.
Keep track of your fluid intake and output, and take note of any decreases in output.
Watch for heart failure symptoms such as increased respiration rate, poor quality of peripheral pulses, chilly extremities and increased breathing rate, decreased LOC, and the appearance of edema. Administer supllemental oxygen via face mask
Place the patient in a semi-fowlers position of comfort.
An increase in the heart rate, fatigue, blood pressure is an indication of complications, cardiac failure.
Reduced cardiac output results in reduced perfusion of the kidneys thus the reduced urine output.
These symptoms indicate a progress in heart failure as the cardiac output decreases.
Helps in increasing ooxygen circulation on the coronary artery thereby good cardiac function.
Elevating the head of the bed helps in decreasing the work of breathing and also reduces the venous return and thus the preload.
Patient was able to perform self-care activities with less fatigue.
Patient maintained normal fluid intake and output.
The vital signs parameters were within the normal ranges indicating an improved cardiac function.
