Evaluation of an Elderly Home Care Program

The program seeks to provide older adults with a home-like environment that provides various services including an opportunity to exercise, nutritional meals, dental care and nutritional training. Also, the facility is expected to deliver oral and dental health services such as tooth extraction, crowns and fill in.     


Non-personnel Resources


The facility is expected to be located in a quite environment with comfortable rooms necessary to accommodate older adults. To help maintain a comfortable living environment at the elderly home care, various resources are essential. The bathrooms are expected to have assistive devices such as shower seats, grab bars and walk-in tubs. For the bedrooms, bed trays, pillows, rails and mattress covers are required. Since the program is expected to have an exercise and fitness center, it is essential for comprehensive fitness gear to be purchased. The program delivers oral and dental healthcare services. Therefore, there is a need for health and medical supplies including daily care accessories. Other equipment to facilitate mobility for older adults is also required in the program. These include walkers, ramps, wheel chairs, and canes. Finally, the program requires safety products such as security devices and equipment are also required in the program. Vision and hearing aids will also be necessary. Such devices include amplified phones to be used by older adults.    


Personnel Resources


The program is expected to hire direct care providers. These employees will be expected to provide attention and personal care to the residents of the facility. The staff includes licensed vocational nurses, certified nursing assistants, physical therapists and registered nurses. The main responsibility of registered nurses is to monitor health and provide medical care at the facility. They are assisted by nursing assistants in the provision of their duties and services. The program will also hire qualified dieticians to identify dietary needs and implement dietary programs at the facility. The facility will also need to employ the administration and support staff tasked with running the program. Such employees include human resource, marketing, and accounting.


Most of the activities at the facility will be managed by direct care providers as recommended by the nurses (Seniors Helping Seniors, 2018). During the day, older adults will be supported to engage in various health and recreation activities. Such activities will be conducted in the morning from 8 am to 11 am. After exercise, older adults will be required to go through bathing and hygiene under the supervision of direct care providers. Other activities include educational training on oral and dental hygiene. These will be conducted from 11 am to 1 pm. The dieticians will be expected to provide information on the nutrition diet options available for older adults at the facility. Such services are to be provided in the afternoon from 2 pm to 4 pm. Medical care including medication will be provided by nurses according to the schedule of individual older adults in the day. Once in every week, the program will facilitate a traveling trip to various areas of interest such as parks. All activities are to be conducted according to the schedule provided by the management of the facility. The management will also allocate duties and responsibilities to the staff employed by the institution. It is also the responsibility of the management to maintain the various equipment at the facility and ensure they are in good form.


The Evaluation Plan


The success of the elderly home project will be measured by looking at the schedule. It is critical for the project to be completed within the set deadline. The scope will also be analyzed through client, employee and stakeholder satisfaction. Other factors used in the evaluation include the budget. The project must be completed within the allocated budget to ensure the business is profitable. Finally, it is essential to look at the quality of care provided at the facility to determine the success of the project. Process evaluation is necessary to assess the quality of the services provided in the program (Sinorita et al., 2016). It is important to track the reach of the services by assessing their implementation and identifying potential problems.


The impact of the project will also be evaluated using the SMART approach to determine whether the program objectives were met. The SMART tool is based on the project being specific, measurable, achievable, and relevant and time specific (Koepke " Fahey, 2016). The assessment will mainly be focused on policy development, service delivery and key areas of knowledge at the elderly care facility. Finally, the outcome will also be evaluated to measure the long-term effects of the services provided by the program. This includes factors such as behavioral change and reduction of incidences by older adults.


For evaluation, both qualitative and quantitative data will be collected. Quantitative data will be collected through surveys, service documents and checklists. On the other hand, qualitative data will be collected using questionnaires, interviews, case studies and focus groups. The data collected includes information on service delivery and client satisfaction. The instruments used in program evaluation includes software that facilitates data analysis and mobile device surveys. The tools were selected because they are unique and specific to the program.
