Employee Volunteer Program and Donation Forms
DR Enterprises will donate 100 dollars to the community for every employee who will volunteer four hours of their free time to serve the public. Therefore, the management requests that all those who want the company to contribute to them to follow the following simple steps to make their applications to the company management.
Step 1: Acquire the Donation form
Acquire a Candles in the Wind donation form (or volunteer form in case one wants to spend their time off helping the needy people in society) from the head of the department on Mondays. The forms are usually available on Mondays between 8.00 am and 3 pm. If for some reason one is unable to get them during the time, they may call the office and ask that their forms be left with the secretary so that they can pick during any other day of the week.
Step 2: Fill out the Form
Fill the form by giving the name of the applicant, the address, and details of the particular community center that one is interested in assisting. One should indicate the given time that they plan to do the community service as well as their work ID number. Also, the details concerning the director or the leader of the voluntary program should be filled to ensure that the follow-up process is smooth.
Step 3: Indicate Volunteer Details
One should also indicate the number of hours that they will be volunteering with the program and what they plan to do during the time.
